Fetcher’s ID Application Form is Now Available for Download
• Parents and guardians may now download the Fetcher’s ID Application Form through our website. This ID is required for authorized individuals who will fetch a student and access the SMCL turnstile.
• Please download the form using the link provided and follow the instructions to ensure proper processing.
• NOTE: Please print on 8.5" X 13" (Long/Folio) paper and back-to-back. Fill in with CAPITAL LETTERS.
Student ID Card Replacement Policy
• If the student ID Card is misplaced, lost, or stolen, he/she must accomplish the following to secure a new ID Card:
1. Notarized affidavit of loss.
2. ID Request Form
3. Official Receipt
• If the student wants to change his/her ID card, the procedure as follows:
1. ID Request Form with old ID card for students
2. Official Receipt
